2009: Colgan Air Flight 3407, a Continental Connection flight, crashes into a house in Clarence Center, N.Y., while on approach to Buffalo-Niagara International Airport, killing all on board and one on the ground. The accident, which triggered a wave of inquiries over the operations of regional airlines in the United States, was later blamed on the pilots' inability to respond properly to stall warnings.
